Answers How care works

What can telehealth actually do for metabolic care, and what still needs to happen in person?

The short answer

Telehealth handles assessment, plan-building, prescribing where state law permits, progress visits and most ongoing management well. What still requires a physical presence is specimen collection for laboratory work, a hands-on physical examination, anything administered by a clinician, and emergency care.

The TelehealthCare delivered remotely, usually by video. In Texas a practitioner-patient relationship can be established this way, within rules set by state law and the medical board.Laboratory draws, hands-on examination and anything a clinician administers still need a physical presence. argument is usually framed as a quality question. It is really a task question: some tasks need a body in a room and most do not.

What telehealth does well

Assessment. A structured intake plus a video conversation covers history, medications, goals, prior attempts and red flags efficiently. Written intake often produces better information than a rushed in-person appointment, because people answer more honestly and more completely when they are not sitting on paper in a gown.

Plan-building. Explaining options, costs, tradeoffs and limits is a conversation. Video is a perfectly good medium for a conversation, and it can be recorded or summarized in writing more easily.

Progress visits. Reviewing response, tolerance and measurements over time is largely a review of information plus a conversation. This is where telehealth is genuinely strong, because a video visit is easier to actually attend, which means it happens.

Ongoing management. Adjustments, pauses, escalation and coordination are administrative and clinical work, not physical work.

What still needs a physical presence

Specimen collection. Blood work requires a draw, whether at a laboratory, through mobile phlebotomy, or at a partner site.

Hands-on examination. Some findings require palpation, auscultation or direct inspection. A clinician who needs one should say so and arrange it.

Anything a clinician administers. If treatment is administered by a clinician rather than self-administered, someone has to be present, and that carries its own credential, delegation, supervision and setting requirements.

Emergencies. Telehealth is never the route for an emergency. Emergency symptoms require emergency services.

The honest failure mode

The failure mode of telehealth is not the camera. It is programs that use remote delivery as a reason to skip the parts that take effort: no scheduled follow-up, no laboratory work, no documented reasoning, no plan for after. That is not a telehealth problem. That is a care-design problem wearing telehealth as a costume.

  • Administration pathway How and where a treatment is actually given: self-administered at home with written guidance, or supported by a scheduled visit from trained staff. Eligibility, credentials and service zones are set by clinical leadership, not by preference.
  • Progress visit A scheduled clinical appointment inside each care cycle where your clinician reviews how you are responding and tolerating treatment, then adjusts, pauses or stops the plan. A support inbox is not a progress visit. This is the step most programs skip.

Related questions

Can a clinician prescribe over telehealth in Texas?
Texas permits the establishment of a practitioner-patient relationship by telemedicine, and prescribing by telemedicine is allowed within specific rules that vary by drug category and circumstance. The specifics are set by state law and board rules, not by a company policy.
Does telehealth mean lower-quality care?
For assessment, planning and ongoing management, the limitation is usually documentation and follow-through, not the video call. A telehealth program with scheduled progress visits and a real record can manage a condition better than in-person care with neither.
